CERTIFIED COPIES OF DEATH CERTIFICATES

Certified copies of the Death Certificates are ordered through the health department where the death occurred. When ordering these copies the health department needs to know the name of the deceased, the location and date of death. The following are the addresses and prices of certified copies for each health department:

For deaths that occurred within the city limits of Cincinnati:
$22.00 for per copy.

SOCIAL SECURITY INFORMATION

Social Security Death Benefit is $255.00, only paid to a surviving spouse, as long as deceased had worked and paid into Social Security for 6 quarters (1 1/2 yrs.)
